Rapptz 6076c8c671 Better chunking behaviour and add members on PRESENCE_UPDATE. 
This should hopefully cover all cases where members are added.
There was a bug where an array of chunks received would get entirely
processed if only a single chunk was received. This was fixed by
explicitly bailing early if we're requesting for chunks.

Rapptz d79a13d7bc [commands] Refactor special cased discord.py converters. 
Code is a little more straightforward. The Member and Channel special
cases will now work in private message contexts when given a name. The
Member special case will also work if you pass in a name and
discriminator combo.

Rapptz 13f8b972e6 Fix bug where the everyone role was not being properly resolved. 
The permissions_for assumed that the everyone role would be the first
element of the permission overwrites but this is not guaranteed so we
have to guarantee it ourselves.

Rapptz 89de0e7e9f Client.send_file can now send content along with the message. 
There is a breaking change here. We have to change the filename
parameter to be a keyword only argument so there are fewer errors
and surprises.

Rapptz 4768d950c5 Offline members are now added by default automatically. 
This commit adds support for GUILD_MEMBERS_CHUNK which had to be done
due to forced large_threshold requirements in the library.

Rapptz 530fbe78b8 Handle case where PRESENCE_UPDATE adds members to cache. 
When we don't request offline members, members that are going from
offline to online in the server will become available via the
PRESENCE_UPDATE event. This should be transparent to the user since
technically the member exists in the server and thus on_member_update
is used instead of on_member_join.

Rapptz 1cd3c0b5cb Make VoiceClient.create_ytdl_player a coroutine. 
This is a breaking change. The reason to make it a coroutine is so that
the info retrieval is not blocking since there are rare instances where
it would take an abnormally long time. The player returned is also
augmented to have more attributes relating to the stream itself.

Rapptz b4604fa3f6 Change options in VoiceClient.create_ytdl_player to ytdl_options. 
This is a breaking change. This allows you to set both ffmpeg options
and regular ytdl options in the same function since we now just
forward the keyword arguments to the ffmpeg player.

Rapptz 1688d513e8 [commands] Context.invoke just invokes a command with no special case. 
Previously it would both forward and invoke depending if there were no
kwargs given. Now it just passes in the arguments to another command
without doing any special casing on the no argument case.

Rapptz 8caadb5f03 [commands] Fix discord.Invite special case handling in parameters. 
This led to decorating a lot of things into @asyncio.coroutine.
Unfortunately there's no way to lower the amount of decoration since
coroutines spread like a virus.

Rapptz bb5e222f29 [commands] Add Bot.help_attrs to customise the help command. 
The help message now uses the invoked_with attribute of the context to
get the name of the command it uses instead of a hardcoded help.

Rapptz abbe79919b [commands] Change Bot.pm_help to be an optional bool. 
The behaviour of passing in pm_help=None makes it so the bot will only
send private messages if the length of the message is too "big", which
is defined as having more than 1000 characters in the output.
